We ask that all of our patients complete this questionnaire 48 hours prior to their appointment. If a patient does not complete this questionnaire prior to their appointment time, we will not be able to see the patient as we will not have had adequate time to screen responses and make necessary calls or further plans to ensure everyone's safety. We will be working hard to contact patients by phone who have not completed this questionnaire online but we ask that you call us at (207) 872-8911 if you have an appointment in less than 48 hours and you have not yet completed the questionnaire online or with us over the phone.
